Te Tumu Waiora

At Silverdale Medical Centre, we offer our patients a service called Te Tumu Wairoa (supported by Procare Fresh Minds), which means ‘to head towards wellness’. Health Improvement Practitioners (HIP’s) and Health Coaches (HC’s) work in practice alongside the GP and wider practice team to provide mental and physical health and wellbeing support. Their care is focused on helping people and their whanau achieve their health and wellbeing goals – using behavioural health strategies to create individualised plans which support self-management.

The HIP and HC are also able to link people with an Awhi Ora community support work if needed.

This service is free for our enrolled patients and the HIP and HC may have appointments available on the day. There are no specific criteria to access this service - anyone of any age with any issue affecting their mental, physical or social wellbeing can access a HIP, and anyone aged 18 years or over who would benefit from lifestyle changes to improve their health and wellbeing can access a HC.
